TEAM REGISTRATION INSTRUCTIONS
To enter into the fall season of Indoor Soccer please complete the roster and either mail or drop off the roster and the entry fee of $500.00 per team (please put ATTN: Jayson on the envelope) See registration instructions here

If you do not have a full roster yet, go ahead and fill out top of sheet and send it with the fee so we can determine how many teams we are going to have. You may add players to your roster at any time.
There will not be any guest players allowed. The coach or manager of a team must have copies of each player's birth certificate on hand in case of the need to prove a players age.

There will be a coaches' meeting TBD at the BankPlus Sports Complex Building. It is very important that each team have only one person there.
